the directors that were elected at the initial shareholders meeting and the
officers that were elected at the Board of Directors meetings

After your shareholders and Board of Directors meetings,
complete this form.
You will need:
- List of corporate officers and addresses
- Number of board vacancies
- Someone in California who is willing to receive the corporation's official mail (agent for
process)

| Lines | What to enter |
| 1 | Corporate name and corporate number stamped on your Articles of
Incorporation.
| | 2 | Street address - do not use P.O. Box. |
| 3 | Leave blank - this is for non-California corporations. |
| 4 | Mailing address. |
| 5-7 | Corporate officers and addresses: you must elect all three
officers, although the same person can hold more than one office. Your
bylaws say that
if you have more than one director, the president and the secretary cannot be the same person.
On the form, do not use post office boxes. |
| 8-10 | Everyone on your Board of Directors - usually the same
people as in lines 5-7 above, plus others on your Board of Directors.
No post office boxes, please. |
| 11 | Board vacancies: If your bylaws authorize more board members, put the number
of vacancies here. |
| 12-13 | Agent: This person was designated on your
Articles of
Incorporation, unless you changed the agent at your Board of Directors meeting. |

If your agent's name or address changes
during the year, you must submit a Statement of Information form
with a fee prior to the corporation's annniversary date. Be sure to record
this in your Board of Director meetings that new agent was appointed.
